Open comedones (blackheads) and closed comedones (whiteheads) form when desquamated keratinocytes mix with oxidized squalene and wax esters, forming dense follicular plugs.

UNDERLYING PATHOPHYSIOLOGY

Biological Mechanism

Follicular retention hyperkeratosis prevents desquamated corneocytes from shedding normally. As sebum pools within the infundibulum, microbial microflora (Cutibacterium acnes) proliferate.
