Biological Mechanism

Unlike Post-Inflammatory Hyperpigmentation (PIH), which is driven by melanocyte melanin synthesis, Post-Inflammatory Erythema (PIE) is a vascular phenomenon. The inflammatory response of an acne papule releases angiogenic cytokines (VEGF) that damage microvascular basement membranes, leaving papillary dermal capillaries chronically dilated and engorged with static deoxygenated blood. Because melanin is not the causative pigment, traditional tyrosinase inhibitors are ineffective; treatment requires vascular stabilization, anti-inflammatory cytokine suppression, and strict non-thermal UV defense.

How do I tell PIE apart from PIH?

Perform the glass slide blanch test: press a clear glass slide firmly against the mark. If the redness disappears momentarily under pressure, it is vascular PIE. If the dark brown spot remains visible under pressure, it is melanin PIH.

Can chemical peels fade PIE?

Generally no. Most medium-depth chemical peels cause inflammatory heating that exacerbates microvascular dilation. Gentle vascular-targeted actives and patience yield superior resolution.
