Biological Mechanism of Irritation

Menthol and camphor are monoterpene counter-irritants that bind to and activate transient receptor potential melastatin 8 (TRPM8) ion channels on cutaneous A-delta and C sensory nerve fibers, causing calcium influx that mimics a physiological temperature drop. However, at higher concentrations, they concurrently activate TRPV1 nociceptors and trigger axon-reflex release of substance P and calcitonin gene-related peptide (CGRP). This neurogenic pathway elicits arterial vasodilation, rebound erythema, and accelerates transepidermal water loss.
