CLINICAL MECHANISM DOSSIER

Rough Texture & Corneocyte Buildup

A rough, tactilely uneven skin surface caused by un-shed corneocytes accumulating on the stratum corneum.

UNDERLYING PATHOPHYSIOLOGY

Biological Mechanism

Normal skin sheds approximately 30,000 to 40,000 dead cells per minute. When desmosomal cadherins fail to detach, corneocytes stack in irregular layers, scattering ambient light and creating tactile roughness. Chemical exfoliants loosen desmosomes to restore uniform planar reflection.

FIRST-LINE EVIDENCE

Primary Recommended Actives

AHA & BHA Acids

ROLE: Desmosomal Cohesion Disruptor
Ingredient profile →

AHAs dissolve calcium bonds between corneocytes; BHA clears follicular duct margins.

Retinoids

ROLE: Keratinocyte Proliferation Stimulator
Ingredient profile →

Accelerates basal layer proliferation, pushing fresh cells upward to create a uniform epidermal architecture.

COMPLEMENTARY MODALITIES

Secondary & Supporting Actives

Niacinamide

Profile →
Keratinocyte Envelope Stabilizer

Improves cellular cohesion and smooths appearance of follicular pores.

CONTRAINDICATIONS & PITFALLS

Ingredients & Approaches to Avoid

Concurrent Retinoid and Glycolic Acid stacking
Simultaneous accelerated shedding outpaces lipid synthesis, causing severe peeling dermatitis.
CADENCE PROTOCOL

AM & PM Application Strategy

MORNING (AM)

Photoprotection & Defense

Gentle hydration + Niacinamide + Sunscreen.

EVENING (PM)

Targeted Cellular Repair

Staggered cadence: Glycolic acid on Monday/Thursday; Retinoid on Tuesday/Friday; barrier recovery on off days.

COMMON INQUIRIES

Frequently Asked Questions

Why do direct acids sting more than retinoids initially?

Direct acids work immediately by lowering surface pH and cleaving desmosomes; retinoids operate via nuclear gene transcription over days and weeks.
